Store Policy

Jex 48 Med Spa – Store Policy 

1. Appointment & Booking Policy
To secure your appointment, a $150 deposit is required at the time of booking. This deposit will be applied toward your treatment total at your visit.

  • No-shows or same-day cancellations forfeit the deposit.
  • Rescheduling requires at least 48 hours’ notice.

2. Cancellations & Refunds
Due to the personalized nature of our services:

  • Deposits are non-refundable in the event of cancellation.
  • No refunds are issued for completed services or packages.
  • Prepaid services or packages may be credited to your account for future use (non-transferable, expires after 12 months).

3. Late Arrivals
Please arrive on time. Arrivals more than 15 minutes late may result in rescheduling and loss of deposit, depending on the provider’s discretion and schedule.

4. Medical Eligibility
All treatments are provided only after medical clearance. Patients must complete a medical intake and may be required to undergo a telehealth or in-person consultation.

  • Jex 48 reserves the right to decline treatment if it is medically inappropriate.
  • Treatments like TRT, GLP-1, Botox, and dermal fillers require approval from a licensed provider.

5. Age Requirement
Patients must be 18 years or older for medical aesthetic treatments unless approved by a legal guardian and under provider discretion.

6. Product Sales
All retail skincare and medical-grade products are final sale. No refunds or exchanges are permitted due to health and safety regulations.

7. Promotions & Discounts
Special offers, discounts, and loyalty rewards cannot be combined, unless explicitly stated.

  • Promotions are subject to change or expire without notice.

8. Payment
We accept major credit/debit cards and electronic payments. Full payment is due at the time of service unless otherwise arranged.

9. Confidentiality & Privacy
We respect your privacy. All patient records are handled in compliance with HIPAA and Arizona state law.
